Transferring your medical record to another clinic

Dr Matt Shaw will be relocating to MyClinic Melbourne from Monday 4 August 2025.

If you book an appointment with him there, Dr Shaw will be able to access your records without any further work.

If you prefer to have your records transferred elsewhere;

  • City Medical will be able to assist until 31 August 2025

  • MyClinic Melbourne will be able to assist at any time.

To have your record sent to another clinic, we request the following process:

  1. Identify your preferred new clinic

  2. Complete and sign a Transfer of Medical Records form at that clinic

  3. Ask them to fax or email that form to City Medical

  4. We will then send your records using their preferred method.
